Cashless in-patient treatment is subject to the approval
from the TPA solely as per their instructions.
The patient or his / her relatives are advised to inform the office regarding Mediclaim
and TPA prior to admission and to get pre-authorisation form. The Pre-authorisation
form should be filled carefully and duly signed by the insured and treating doctor.
In case patient fails to inform the TPA/Insurance Company within the stipulated
period or there is a denial from the TPA/Insurance Company, the patient will be
treated as normal cash paying patient.

- Policy Copies
- Id Proof of Patient
- If corporate policy: Corporate id card
- Positive Investigation reports
- Doctor's Consultation papers
- MLC/FIR copy,previous discharge card:if applicable for particular case
